When Additional Care is Needed...

Intermediate Care

When You Need the Assurances of Extra Care

Fair Hills Residence is licensed by the State of Illinois to provide intermediate care services. When a resident's care is beyond assisted living services, intermediate care can provide the extra nursing and supportive services necessary for those recovering from an illness (short term) or to provide supervision for those with chronic medical problems (long term). Intermediate Care is appropriate for persons who may need some nursing intervention, but not on a continuous basis. Rehabilitation programs, activities and personal care assistance are available, and the care is provided under the direction of a physician.

Rooms in our Care Center include:

  • Supervised nursing care on a daily basis from a written plan of care.

  • Care for the semi-independent residents who require the assist of one person for transfers.

  • Medication management supervised by a nurse following a doctor's order.

  • Hands-on minimal assistance with activities of daily living (ADL), including significant assistance with up to two ADL’s.

  • Resident and family participation in developing their care plan

  • Private Dining Room with assistance as needed.
